This form is used to ask about food allergies in advance.
It is not intended to ask about personal preferences or dislikes.
We appreciate your cooperation in order to ensure your food safety.

At our establishment, we use the same kitchen, cooking utensils, washing equipment, and tableware for all menu items.
Therefore, there is a possibility that trace amounts of allergens may be unintentionally mixed in during the preparation process (cross-contamination).

Please note the following in advance:

  • We do not accommodate seasoning adjustments (e.g., reduced salt, stronger flavoring, etc.).
  • We do not provide Muslim-friendly meal preparation.
  • If we determine that it is difficult to safely provide allergy-friendly meals, we may decline to serve meals in order to prioritize guest safety.

This form will be used solely as reference material to ensure that guests with food allergies can stay and dine with peace of mind.It will not be used for any other purpose. Personal information will be handled responsibly and stored/disposed of with care.

Please notify us of any food allergies in advance (at least 3 days prior to your stay) by email (using this form).We are unable to accommodate allergy requests on the day of your stay due to food safety considerations and to prevent loss of prepared ingredients.
